В исследование включены 60  пациентов, которым в период с 2012 г. на базе ФГБУ «НМИЦ им. акад. Е.Н. Мешалкина» Минздрава России выполнена ОТС. Проведена оценка влияния длительности холодовой ишемии трансплантата на исход ОТС в отдаленном периоде  наблюдения. Оценивали выживаемость, качество жизни по данным опросника SF-36, параметры  кардиопульмонального нагрузочного тестирования (КПНТ).</p></sec><sec><title>Результаты</title><p>Результаты. Длительность холодовой ишемии  трансплантата в изучаемой группе составила 210 (175–340)  мин; минимальная длительность – 158 мин, максимальная –  430 мин. Ишемия трансплантата более 240 мин  зарегистрирована в 42% случаев. В отдаленном периоде  наблюдения выживаемость достигала 87%. Толерантность к  физической нагрузке пациентов соответствовала уровню  выше среднего или высокому: пиковое потребление  кислорода равнялось 17 (14,7–21,0) мл/мин/кг при пороговой мощности нагрузки 100 (90–120) Вт. Уровень качества жизни по физическому компоненту здоровья опросника SF-36 составлял 53 (50–55) балла, по  психоэмоциональному компоненту здоровья – 52 (50–56) балла. Значимых ассоциаций длительности холодовой  ишемии трансплантата с выживаемостью, уровнем качества жизни и параметрами КПНТ не выявлено.</p></sec><sec><title>Заключение</title><p>Заключение. Длительность холодовой ишемии  трансплантата не показала отрицательного влияния на  результаты ОТС в отдаленном периоде наблюдения. Изучаемая группа реципиентов характеризовалась высокой  выживаемостью и высокой толерантностью к физическим нагрузкам в отдаленные сроки после ОТС. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title> Aim</title><p> Aim. To evaluate the effect of the duration of graft cold ischemia on the long-term prognosis of orthotopic heart transplantation (OHT).</p></sec><sec><title>Material and Methods</title><p>Material and Methods. The study included 60 patients, who had receieved OHT in the Meshalkin National Medical Research Center since 2012. The influence of the duration of graft cold ischemia on the outcome of OHT in the long-term follow-up was evaluated. Survival, quality of life according to the SF-36 questionnaire, and parameters of cardiopulmonary exercise testing (CPET) were assessed.</p></sec><sec><title>Results</title><p>Results. The duration of graft cold ischemia in the study group was 210 (175–340) minutes. The minimum duration time was 158 minutes; the maximum duration time was 430 minutes. Graft ischemia for more than 240 minutes was registered in 42% of cases. The survival rate was 87% in the long-term follow-up. Exercise capacity of patients corresponded to the level above average or high: peak oxygen consumption was 17 (14.7–21.0) mL/min/kg with a threshold load power of 100 (90–120) W. The levels of quality of life based on the SF-36 questionnaire scoring were 53 (50–55) points for the physical component of health and 52 (50–56) points for the psycho-emotional component of health. No significant associations of graft cold ischemia duration were found with the survival rate, quality of life, and CPET parameters.</p></sec><sec><title>Conclusion</title><p>Conclusion. The duration of graft cold ischemia did not show any negative effect on the results of OHT in the long-term followup.
