В проспективное исследование включены 57 пациентов, оперированных по поводу аневризм и расслоений грудной аорты. Пациенты были разделены на две группы. Основной группе больных (n = 27) проводили комплексную защиту легких в виде перфузии легочной артерии и искусственной вентиляции легких (ИВЛ). Пациентам контрольной группы (n = 30) мероприятия по комплексной защите легких не проводили.</p></sec><sec><title>Результаты</title><p>Результаты. Медианная продолжительность ИВЛ в послеоперационном периоде составила 14 ч в основной группе и 23 ч – в контрольной группе больных (p &lt; 0,001). В раннем послеоперационном периоде не было получено значимой разницы между группами по частоте развития анализируемых легочных осложнений. Число пневмоний было 8 (14%), из них в основной группе – 4 (15%) случая и контрольной группе – 4 (13%) эпизода (p = 0,87).</p></sec><sec><title>Выводы</title><p>Выводы. Селективная перфузия легочной артерии (СПЛА) в сочетании с ИВЛ редуцированными объемами при операциях на грудной аорте в условиях ЦА значимо снижает длительность послеоперационной ИВЛ. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title> Aim</title><p> Aim. The aim of the study was to analyze the effectiveness of  comprehensive lung protection in patients who underwent thoracic aortic surgery using circulatory arrest.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. A total of 57 patients undergoing operations for aneurysms and thoracic aortic dissection were enrolled in the prospective study. The patients were divided into two groups. The experimental group of patients (n = 27) underwent comprehensive lung protection with pulmonary artery perfusion and mechanical ventilation. Patients in the control group (n = 30) did not undergo comprehensive lung protection measures.</p></sec><sec><title>Results</title><p>Results. The median duration of mechanical ventilation in the postoperative period was 14 hours in the experimental group and 23 hours in the control group of patients (p &lt; 0.001). The incidence of analyzed pulmonary complications did not significantly differ between the groups during the early postoperative period. The number of pneumonia cases was 8 (14%) including 4 (15%) cases in the experimental group and 4 (13%) episodes in the control group (p = 0.87).</p></sec><sec><title>Conclusion</title><p>Conclusion.
