Обследовано 423 пациента с различными формами ФП, медикаментозно резистентных к антиаритмической терапии. Всем пациентам проводилось внутрисер-дечное электрофизиологическое исследование сердца (ВСЭФИ). В зависимости от электрофизиологических форм ФП выбран тип оперативного лечения для каждого пациента. Электрофизиологическое выявление формы Фп позволяет разделить подходы катетерного лечения. В основе катетерного лечения фокусной формы ФП лежит радиочастотная изоляция аритмогенной области. В случае выявления участков замедления (аритмогенных зон) в миокарде левого предсердия, способствующих развитию механизма повторного входа, необходимо проведение дополнительной дефрагментации этого участка.</p></abstract><trans-abstract xml:lang="en"><p>The objective of the study was to assess the efficacy of various approaches for surgical treatment of atrial fibrillation (AF), depending on the electrophysiological mechanisms. A total of 423 patients with various forms of AF resistant to antiarrhythmic drug therapy were surveyed. All patients underwent intracardiac electrophysiological study. Depending on the electrophysiological forms of AF, the type of surgical treatment for each patient was selected. Electrophysiological detection of AF provides a way to differentiate the approaches for catheter treatment. Catheter treatment of focal AF is based on radiofrequency isolation of the arrhythmogenic region.
