Однако, несмотря на эти значительные успехи, фибрилляция желудочков (ФЖ) или желудочковая тахикардия (ЖТ) все еще имеют место во время острой фазы ИМ в 2-8% случаев. Постинфарк-тные желудочковые аритмии (ЖА) традиционно делится на два типа: ранние ЖА происходят в течение первых 48 ч после появления симптомов острого ИМ, в то время как поздние возникают позднее 48 ч. Ранние ЖА часто связаны с транзиторной ишемией. Несколько исследований показали, что пациенты с ФЖ во время острой фазы ИМ имеют более высокий риск смерти в краткосрочном периоде. В отличие от этого данные о выживаемости в средней и долгосрочной перспективе являются ограниченными и спорными. Поздние ЖА, в частности устойчивая мономорфная желудочковая тахикардия, возникают наиболее часто из-за рубцовой ткани и механизма повторного входа возбуждения. Такой аритмогенный субстрат считается необратимым и, согласно рекомендациям, есть общее соглашение, что пациентам, пережившим поздние ЖА при ИМ, следует имплантировать кардиовертер-дефибриллятор (ИКД) до выписки из стационара, в то время как полезность ИКД у пациентов с ранними ЖА до сих пор дискутабельна и менее определена.</p></abstract><trans-abstract xml:lang="en"><p>The prognosis following acute myocardial infarction (MI) has improved considerably in recent decades, essentially due to the development of percutaneous coronary intervention and improved medical therapy, including administration of thrombolytic agents. However, despite these major advances, ventricular fibrillation (VF) or rapid ventricular tachycardia (VT) still occur during the acute phase of MI in 2-8% of cases. Post-MI VA is traditionally divided into two types: early VA occurs within 48 h of MI symptom onset, whereas late VA occurs over 48 h after MI. Early VA is often attributed to the transient ischaemic settings. Several studies have shown that patients who develop VF during the acute phase of MI have a higher risk of death in the short-term. In contrast, data on mid-to-long-term survival are limited and controversial. Late VA, particularly sustained monomorphic ventricular tachycardia (VT), is more frequently due to a reentrant mechanism resulting from the presence of scar tissue, and the arrhythmogenic substrate is thought to be irreversible in these patients.
