Течение острого коронарного синдрома (ОКС) часто сопровождается ишемическим повреждением головного мозга вследствие резкого нарушения сократительной функции миокарда, иногда в результате остановки сердца. Гипоксическое повреждение головного мозга может проявляться ишемическим инсультом или психосоматическими расстройствами, такими как делирий, депрессивные или тревожные расстройства. Однако универсального маркера повреждения головного мозга не найдено. Одним из кандидатов на лабораторную диагностику повреждения головного мозга является белок S100B.Цель: изучить уровень мозгоспецифического белка S100B в плазме крови у пациентов с ОКС, ассоциированным с тревожно-депрессивными расстройствами (ТДР).Материал и методы. В исследование был включен 81 пациент с ОКС и наличием ТДР. Пациентам было проведено анкетирование с помощью госпитальной шкалы тревоги и депрессии (ГШТД), шкалы депрессии Бека, шкалы тревоги Шихана. Всем пациентам проводился забор крови на исследование уровня белка S100B.Результаты. У пациентов, включенных в исследование, было диагностировано наличие клинически выраженной тревоги и субклинически выраженная депрессия. У 35 пациентов (43%) уровень белка S100В был в диапазоне от 0 до 90 нг/л, а у 46 пациентов (57%) – выше 90 нг/л. При проведении корреляционного анализа у пациентов выявлены положительные корреляции между высоким уровнем белка S100B в сыворотке крови и клиническими показателями (сахарным диабетом, ожирением, курением, артериальной гипертензией (АГ), постинфарктным кардиосклерозом, мозговым инсультом в анамнезе), уровнем тревоги и депрессии, а также отрицательные корреляции между высоким уровнем белка S100B в сыворотке крови и скоростью клубочковой фильтрации, фракцией выброса левого желудочка.Заключение. Повышение уровня белка S100В отмечалось у коморбидных пациентов с клинически выраженной тревожно-депрессивной симптоматикой. Возможно, более высокие значения глиального белка S100B в крови при поступлении у пациентов с ОКС могут предсказывать более тяжелое течение сердечно-сосудистой патологии и худший неврологический прогноз, но это требует дальнейшего изучения.</p></abstract><trans-abstract xml:lang="en"><p>Background. Acute coronary syndromes are often associated with the onset or aggravation of anxiety and depressive disorders, delirium, cerebral ischemia up to the development of a stroke.Aim: To study the level of brain-specific protein S100B in blood plasma in patients with acute coronary syndrome (ACS) associated with anxiety-depressive disorders (ADD).Material and Methods. The study included 81 patients with ACS and the presence of anxiety and depression. Patients were surveyed using the Hospital Anxiety and Depression Scale, the Beck Depression Inventory, and the Sheehan Patient-Rated Anxiety Scale. All patients underwent blood sampling to study the level of S100B protein.Results. Patients included in the study were diagnosed with clinical anxiety and subclinical depression. In 35 patients (43%), the S100B protein level varied from 0 to 90 ng/l, and in 46 patients (57%), its level was above 90 ng/l. According to correlation analysis in patients, positive correlations were found between a high level of S100B protein in the blood serum and clinical indicators (diabetes mellitus, obesity, smoking, arterial hypertension, postinfarction cardiosclerosis, a history of cerebral stroke), levels of anxiety and depression, as well as negative correlations between high level of S100B protein in blood serum and glomerular filtration rate, left ventricular ejection fraction.Conclusions. An increase in the level of S100B protein was observed in comorbid patients with clinically pronounced anxiety and depressive symptoms.
