Томска. В рамках одномоментного поперечного исследования (cross-sectional study) неорганизованной популяции 25-64 лет г. Томска (n=1600 чел.) выполнено скрининговое ультразвуковое исследование экстракраниального каротидного бассейна на предмет выявления атеросклеротических бляшек (АСБ). В качестве потенциальных детерминант, обусловливающих закономерности встречаемости АСБ в обследованной группе населения, исследовали возраст, пол, стаж курения, общий холестерин (ОХС), липопротеиды высокой плотности (ЛПВП), систолическое артериальное давление (САД), абдоминальное ожирение (АО), низкий уровень образования, высокочувствительный С-реактивный белок, наличие сахарного диабета, уровень глюкозы, гипотензивную терапию, гиполипидемическую терапию. Для выявления ассоциаций использовали логистический регрессионный анализ. Вероятность ошибки менее 5% считали статистически значимой. В рамках однофакторного анализа все исследуемые факторы показали взаимосвязь с распространенностью АСБ на высоком статистическом уровне значимости. После поправки на влияние возраста и пола только ОХС, стаж курения и низкий уровень образования были связаны с встречаемостью АСБ. По данным многомерного анализа, выявлено 8 факторов, независимо ассоциированных с распространенностью КАС в обследованной популяции. Среди них возраст, ОХС, стаж курения, ЛПВП и мужской пол были наиболее влиятельными детерминантами. Полученные результаты свидетельствуют о целесообразности более эффективного контроля целевых параметров липидного профиля, прежде всего ОХС, а также предупреждения экспозиции курения для снижения распространенности КАС среди городского взрослого населения, проживающего в условиях сибирского региона.</p></abstract><trans-abstract xml:lang="en"><p>The aim of the study was to investigate the multifactorial associations between the traditional cardiovascular risk factors and prevalence of carotid atherosclerosis in general population of Tomsk. Cross-sectional epidemiologic study of general Tomsk population aged 25-64 years (n=1600) consisted in screening examination of extracranial carotid vascular bed to detect the presence of atherosclerotic plaques by ultrasonography. We examined age, gender, smoking exposure duration, total cholesterol, high density cholesterol, abdominal obesity, low educational status, systolic blood pressure, hsCRP, fasting plasma glucose, diabetes mellitus, antihypertensive treatment, and cholesterol-lowering treatment as potential determinants of carotid atherosclerosis. To study associations, we used logistic regression analysis. Error probability &lt;5% was considered as statistically significant. Single-factor analysis showed that all factors were significantly associated with carotid plaque prevalence. After adjustment for age and gender, only three factors, namely: total cholesterol, smoking exposure duration, and low education were significantly associated with carotid plaques. Multiple regression analysis showed that eight factors correlated with carotid plaque prevalence in an independent manner. Among these factors, the most influential determinants of carotid plaques were age, total cholesterol, smoking exposure duration, high density cholesterol, and gender.
