В настоящий момент существует два основных подхода: стентирование одним длинным стентом (ОДС) и стентирование двумя и более стентами с перекрытием (СП). Однако ввиду конической геометрии сосуда стентирование ОДС при большой разнице между проксимальным и дистальным референсными диаметрами артерии может привести к худшим результатам, чем стентирование СП за счет более низкой комплаентности устройств к сосуду. В статье представлен клинический случай, иллюстрирующий предлагаемый нами алгоритм принятия решения в пользу ОДС или СП на основе данных, полученных при помощи оптической когерентной томографии (ОКТ).</p></abstract><trans-abstract xml:lang="en"><p>One of the topical issues in percutaneous coronary interventions (PCI) is the stenting of extended lesions. Currently, there are two main approaches: stenting with a single long stent (SLS) and stenting with two or more overlapping stents (OS). However, due to the conical geometry of the vessel, stenting of the SLS with a large difference between the proximal and distal reference diameters of the artery may lead to worse results than stenting of the OS, due to lower device compliance to the vessel.
