Пациенты рандомизированы в две группы. Всем пациентам проводилась магнитно-резонансная томография (МРТ) с фармакологическим стресс-тестом для оценки перфузии миокарда. Пациентам группы 1 выполнялась эндоваскулярная реканализация ХО ПКА, группа 2 получала только консервативную терапию. Период наблюдения составил 2 мес. Все пациенты были классифицированы как имеющие высокий риск ишемии миокарда (≥2 сегментов). Через 2 мес. рандомизации в группе 1 отмечено снижение показателя количества сегментов с дефектами перфузии (-2,05 сегментов, 95% ДИ для разницы -2,58...-1,52, р&lt;0,001) и достоверное снижение количества пациентов с высоким риском ишемии (≥2 сегментов) с 39 (100,0%) до 29 (74,4%), р&lt;0,001 по сравнению с группой 2, в которой не было отмечено достоверного снижения в динамике. Полученные данные продемонстрировали значимость метода МРТ с фармакологическим стресс-тестом у данной категории пациентов. Таким образом, чрескожное коронарное вмешательство (ЧКВ) при ХО ПКА достоверно снижает риск ишемии миокарда по данным стресс-МРТ. МР-картина перфузии миокарда у пациентов без ЧКВ достоверно не меняется.</p></abstract><trans-abstract xml:lang="en"><p>The following research paper introduces the results of prospective randomized study comprising 72 patients with chronic total occlusion of the right coronary artery (RCA CTO). Participants were divided in 2 groups. All patients underwent cardiac magnetic resonance imaging (CMRI) with pharmacological stress-test for the evaluation of myocardial perfusion. Group 1 received endovascular recanalization of RCA CTO; group 2 received standard medical treatment. The follow-up examinations were performed at 2 and 12 months. All participants were evaluated as myocardium ischemia high risk patients (≥2 segments affected). After 2-month post randomization, statistically significant decrease in the number of segments with perfusion defects (-2.05 segments ranging from -2.58 to -1.52, 95% CI, р&lt;0.001) and reduction in the number of high risk patients (from 39 (100.0%) to 29 (74.4%), р&lt;0.001), were detected in group 1 compared with group 2. Neither of parameters in group 2 changed significantly at 1 year follow up.
