В исследование включались пациенты с «неклапанной» ФП с ТЭО в анамнезе и выявленными тромбозами на фоне получаемой регулярно терапии антикоагулянтами в период 01.2020–01.2023 гг.: 13 пациентов c анамнестическими указаниями на перенесенные ТЭО (группа 1), 18 – с первично диагностированными тромбозом / спонтанным эхоконтрастированием предсердий высокой степени (группа 2). Группой сравнения были здоровые добровольцы (n = 31) (группа 3). Проводилось изучение спонтанной агрегации тромбоцитов и стимулированной индукторами (диагностические наборы с растворами АДФ, адреналина и коллагена), определение уровня маркеров CD40, растворимого лиганда CD40 (CD40L).</p></sec><sec><title>Результаты</title><p>Результаты. Степень спонтанной агрегации тромбоцитов по кривой светопропускания была значимо выше у пациентов группы 2 в сравнении со здоровыми добровольцами с трендом образования агрегатов большего размера у пациентов с ТЭО. При индукции агрегации коллагеном скорость образования агрегатов была значительно ниже в группах 1 и 2 в сравнении со здоровыми добровольцами. При использовании АДФ отмечено сопоставимое снижение степени и скорости, а при использовании адреналина – только скорости агрегации тромбоцитов, оцениваемой по кривой светопропускания, в группах 1 и 2. Концентрация растворимого CD40L значимо преобладала у пациентов с тромботическими осложнениями и ТЭО в анамнезе над таковой у здоровых добровольцев.</p></sec><sec><title>Выводы</title><p>Выводы. Спонтанная агрегационная активность тромбоцитов и концентрация растворимого CD40L повышена как у пациентов с перенесенными ТЭО, так и у пациентов с вновь диагностированными тромбозами на фоне ФП и постоянной монотерапии антикоагулянтами.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim: To study spontaneous and stimulated by adenosine diphosphate (ADP), adrenaline and collagen platelet aggregation activity, CD40-CD40 ligand system in patients with atrial fibrillation (AF) with previous thromboembolic complications (TEC) and newly diagnosed thrombosis.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. The study included patients with «non-valvular» AF and anamnesis of TEC and newly identified thrombosis on anticoagulants in the period 01.2020-01.2023: 13 patients had anamnesis of TEC (group 1), in 18 patients were initially diagnosed thrombosis/spontaneous atrial echocontrast of high grade (group 2). The comparison group was the healthy individuals (n = 31) (group 3). The spontaneous aggregation ability of platelets and stimulated with solutions of ADP, adrenaline and collagen, the level of CD40 markers, soluble CD40 ligand (CD40L) were studied.</p></sec><sec><title>Results</title><p>Results. The spontaneous platelet aggregation activity was significantly higher in patients of group 2 compared with healthy individuals with a formation trend of larger aggregates in patients with TEC. When aggregation was induced by collagen, the rate of aggregate formation was significantly lower in the 1st and 2nd groups compared with healthy individuals. When using ADP, a comparable decrease in the degree and speed was noted, and when using adrenaline, only the rate of platelet aggregation was observed in groups 1 and 2. The concentration of soluble CD40L was significantly higher in patients with a history of TEC and newly thrombosis.</p></sec><sec><title>Conclusion</title><p>Conclusion.
