При выборе метода лечения идиопатических ЖА все чаще отдается предпочтение радиочастотной катетерной аблации (РЧА) перед консервативной терапией. Отдаленная эффективность РЧА у детей с ЖА достигает 87%. У детей с идиопатическими ЖА предикторы риска неэффективной РЧА остаются неизвестными.</p><sec><title>Цель исследования</title><p>Цель исследования: выявить предикторы и построить модель прогнозирования неэффективности РЧА у детей с идиопатическими ЖА в отдаленном периоде.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В ретроспективное исследование включены 164 пациента в возрасте от 7 до 17 лет с идиопатическими ЖА, которым была выполнена РЧА в ФГБУ «НМИЦ им. В.А. Алмазова» с 2011 по 2023 гг. Результаты аблации оценивались в остром периоде и спустя 6 мес. после операции.</p></sec><sec><title>Результаты</title><p>Результаты. Острая эффективность РЧА составила 92,0%, отдаленная – 88,4%. Построена модель бинарной логистической регрессии, прогнозирующая вероятность неэффективной РЧА в отдаленном периоде (p &lt; 0,001). Категориальные предикторы, включенные в модель: наличие полифокусной ЖА (ОШ: 8,814; 95% ДИ: 1,943–39,981; p = 0,005), предшествующая эффективность антиаритмической терапии (ААТ) (ОШ: 16,522; 95% ДИ: 1,959–139,374; p = 0,010), отсутствие индукции ЖА в ходе РЧА (ОШ: 9,369; 95% ДИ: 1,400–62,684; p = 0,021). Чувствительность и специфичность модели составили 73,3 и 74,1% соответственно.</p></sec><sec><title>Выводы</title><p>Выводы. Нами были выявлены такие независимые предикторы неэффективной катетерной аблации у детей с идиопатическими нарушениями ритма сердца, как наличие полифокусной ЖА, предшествующая эффективность ААТ, отсутствие индукции ЖА в ходе операции. Наличие двух и более зон желудочковой эктопии и эффективность проводимой ААТ могут быть приняты во внимание уже на этапе отбора пациентов для проведения РЧА.</p></sec></abstract><trans-abstract xml:lang="en"><p>Ventricular arrhythmias (VAs) in children are generally idiopathic and have a benign course. Catheter ablation (CA) is increasingly becoming a first-line treatment option in children with idiopathic VAs. Acute ablation success reaches 100% procedures and long-term – 87%. Predictors of outcomes of CA in pediatric patients with idiopathic VAs have not been fully investigated.</p><sec><title>Aim</title><p>Aim: To identify predictors of long-term unsuccessful outcomes of CA in pediatric idiopathic VAs.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. This retrospective study included 164 children aged 7 to 17 years with idiopathic VAs, who underwent CA from 2011 until 2023 in Almazov National Medical Research Centre. Efficiency was assessed in acute period and 6 months after CA.</p></sec><sec><title>Results</title><p>Results. In this study the acute success was 92% and long-term success was 88,4%. In multivariable models, multifocal VAs (odds ratio: 8,814; 95% CI: 1,943–39,981; p = 0,005), effectiveness of antiarrhythmic therapy (odds ratio: 16,522; 95%CI: 1,959–139,374; p = 0,010) and noninducibility of VAs during procedure (odds ratio: 9,369; 95% CI: 1,400–62,684; 0,021), become predictors of long-term unsuccessful outcomes of CA in pediatric idiopathic VAs.</p></sec><sec><title>Conclusion</title><p>Conclusion. In our study there were identified independent predictors of long-term noneffective CA in children with idiopathic VAs, such as multifocal VAs, effectiveness of antiarrhythmic therapy, and noninducibility of VAs CA.
