Общий психологический статус и эмоциональное состояние пациентов, страдающих нарушениями ритма сердца, играют важную роль в протекании до- и послеоперационного периода. При этом психологические эффекты подхода fast track в кардиохирургии, в отличие от клинических эффектов, изучены недостаточно.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: определение динамики эмоционального состояния (выраженности симптомов тревоги, депрессии и стресса) пациентов с нарушениями ритма сердца в периоперационном периоде при проведении радиочастотной аблации (РЧА) по системе fast track, а также сопоставление показателей пациентов с нормативными значениями.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Исследованы 77 больных с нарушениями ритма сердца (средний возраст – 57,09 ± 13,606 года; 44,2% мужчины), госпитализированных для проведения РЧА. Применялись психодиагностические опросники: шкала депрессии, тревоги и стресса (DASS-21) и шкала воспринимаемого стресса (ШВС-10).</p></sec><sec><title>Результаты</title><p>Результаты. Установлено, что предоперационная тревога (от умеренной до очень высокой) наблюдается у 57,9% пациентов, признаки эмоционального стресса – у 34,2%, симптомы депрессии – у 18,4%. При этом симптомы тревоги и стресса перед РЧА существенно превосходят нормативные показатели (p ≤ 0,001). После РЧА выраженные признаки тревоги наблюдаются у 44,7%, стресса – у 26,9%, депрессии – у 13,5% пациентов; послеоперационные показатели тревоги и стресса также превосходят нормативные (p ≤ 0,001). Статистически значимой динамики неблагоприятных эмоциональных показателей после проведения РЧА выявлено не было, отмечено лишь снижение эмоционального стресса – на уровне статистической тенденции (p &lt; 0,1).</p></sec><sec><title>Заключение</title><p>Заключение. Целесообразно проведение более дифференцированного исследования эмоционального состояния пациентов, проходящих через РЧА, на расширенной выборке. Пациентам с нарушениями ритма сердца рекомендуется психологическое сопровождение в периоперационном периоде в связи с высокой стрессогенностью оперативного вмешательства и их эмоциональной нестабильностью.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. The psychological status and emotional state of patients suffering from heart rhythm disorders play an important role in the course of the pre- and postoperative period, whereby psychological effects of the fast-track approach in cardiac surgery, in contrast to clinical effects, are insufficiently studied.</p></sec><sec><title>Aim</title><p>Aim: To investigate the dynamics of patients’ emotional state (severity of anxiety, depression and stress symptoms) in the perioperative period during fast track radiofrequency ablation (RFA), as well as to compare it with the normative values.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. The sample consisted of 77 patients with heart rhythm disorders (mean age 57.09 ± 13.606; 44.2% male) hospitalised for RFA; psychodiagnostic questionnaires DASS-21 and PSS-10 were used.</p></sec><sec><title>Results</title><p>Results. It was found that preoperative anxiety (from moderate to very high) was observed in 57.9% of patients, symptoms of emotional distress – in 34.2%, symptoms of depression – in 18.4%; at the same time symptoms of anxiety and stress before RFA significantly exceeded the normative values (p ≤ 0.001). After RFA, pronounced symptoms of anxiety are observed in 44.7% of patients, stress – in 26.9%, depression – in 13.5% of patients; postoperative anxiety and stress symptoms also exceed the normative values (p ≤ 0.001). No statistically significant dynamics of unfavourable emotional indicators after RFA was revealed, only a decrease in emotional stress was noted, at the level of statistical trend (p &lt; 0.1).</p></sec><sec><title>Conclusion</title><p>Conclusion. It is expedient to carry out a more differentiated study of the emotional state of patients undergoing RFA on an expanded sample.
