Развитие малоинвазивной хирургии и публикация отдаленных положительных результатов способствуют расширению ее применения. По сравнению со стандартным срединным стернотомным доступом минимально инвазивные вмешательства на клапанах сердца обеспечивают меньшую кровопотерю, снижение послеоперационных болей и осложнений, а также ускоренное восстановление с сокращением пребывания в отделении реанимации и интенсивной терапии (ОРиИТ) и в стационаре. Несмотря на отсутствие отечественных публикаций, в мире описаны отдельные случаи коррекции митрального и аортального клапанов через правостороннюю мини-торакотомию. В настоящей работе представлен опыт успешного одномоментного протезирования обоих клапанов через данный доступ у 53-летней пациентки.</p></sec><sec><title>Цель</title><p>Цель: оценить возможность мини-инвазивных технологий (правосторонней мини-торакотомии) в хирургическом лечении пациентов с сочетанным поражением митрального и аортального клапанов.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В клинике сердечно-сосудистой хирургии Первого МГМУ им. И.М. Сеченова (Москва) был опробован новый доступ – правосторонняя мини-торакотомия – для лечения пациентки 53 лет с сочетанным поражением митрального и аортального клапанов. С учетом ее профессиональной деятельности (спорт) принципиальным было сохранение целостности грудной клетки и отказ от стернотомии. Примененный метод позволил снизить риск осложнений и ускорить восстановление.</p></sec><sec><title>Результаты</title><p>Результаты. Операция продемонстрировала технические особенности и преимущества мини-инвазивного подхода. Правосторонняя торакотомия при реконструкции двухклапанных пороков позволяет снизить операционную травму, частоту дыхательных и инфекционных осложнений, кровопотери, послеоперационную боль и длительность искусственной вентиляции легких. Отмечены более редкие случаи фибрилляции предсердий и лучший косметический результат. Существенным преимуществом также является сокращение пребывания в ОРиИТ и стационаре, что снижает затраты на лечение.</p></sec><sec><title>Заключение</title><p>Заключение. Методика коррекции двухклапанного порока через правостороннюю мини-торакотомию не вызывает затруднений при наличии опыта в одноклапанных вмешательствах. Полученные результаты сопоставимы с таковыми при стандартной стернотомии. Для объективной оценки преимуществ необходимы рандомизированные многоцентровые исследования.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The development of minimally invasive surgery and the publication of long-term favorable outcomes contribute to the expanding use of these techniques. Compared to the conventional median sternotomy, minimally invasive valve surgery offers reduced blood loss, less postoperative pain and complications, and faster recovery with shorter stay in the ICU and hospital. Despite the lack of domestic publications, individual cases of mitral and aortic valve repair via right mini-thoracotomy have been described around the world. This paper presents the experience of a successful simultaneous prosthetic replacement of both valves through this approach in a 53-year-old female patient.</p></sec><sec><title>Aim</title><p>Aim: To evaluate the feasibility of using minimally invasive techniques (right mini-thoracotomy) for surgical treatment of combined mitral and aortic valve disease.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. At the Department of Cardiovascular Surgery of Sechenov University (Moscow), a novel approach – right mini-thoracotomy – was tested for treating a 53-year-old female patient with combined mitral and aortic valve pathology. Given her professional involvement in sports, preserving the integrity of the chest wall and avoiding sternotomy was a key consideration. This method helped reduce complication risks and accelerated recovery.</p></sec><sec><title>Results</title><p>Results. The surgery demonstrated technical features and advantages of the minimally invasive approach. Right mini-thoracotomy in double valve surgery reduces surgical trauma, the incidence of respiratory and infectious complications, bleeding, postoperative pain, and the duration of mechanical ventilation. A lower rate of atrial fibrillation and improved cosmetic outcomes were also observed. An additional benefit was the shorter ICU and hospital stay, resulting in decreased treatment costs.</p></sec><sec><title>Conclusion</title><p>Conclusion. Right mini-thoracotomy for double valve surgery poses no significant difficulties when the surgeon is experienced in single-valve interventions. The outcomes are comparable to those of the standard sternotomy approach.
