На сегодняшний день радионуклидная визуализация соматостатиновых рецепторов 2-го типа (SSTR-2) имеет большие перспективы в качестве нового маркера сердечно-сосудистого воспаления. Однако до сих пор остается неизвестным, является ли локальное воспаление, а также высокое содержание макрофагов в зоне инфаркта, экспрессирующих на своей мембране SSTR-2, положительным или отрицательным фактором для заживления миокарда.</p></sec><sec><title>Цель</title><p>Цель: оценка взаимосвязи биомаркеров воспаления как показателей системного ответа на ишемическое повреждение и локального воспалительного ответа в миокарде, измеренного с помощью SSTR-2-направленной визуализации в раннем постинфарктном периоде.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование вошли 23 пациента с острым первичным инфарктом миокарда передней стенки и подъемом сегмента ST (STEMI). В 1-е сут (до проведения чрескожного вмешательства (ЧКВ)) и на 5-е сут после острого инфаркта миокарда (ОИМ) у всех пациентов проводился забор венозной крови для определения уровня высокочувствительного С-реактивного белка (вч-СРБ) и интерлейкина-6 (ИЛ-6). На 5–6-е сут после развития острого коронарного события всем пациентам была выполнена однофотонная эмиссионная компьютерная томография (ОФЭКТ) / компьютерная томография (КТ) с 99mTc-Тектротидом, а на 7-е сут после развития острого коронарного синдрома (ОКС) – перфузионная сцинтграфия миокарда с 99mTc-Технетрилом в покое.</p></sec><sec><title>Результаты</title><p>Результаты. Согласно результатам исследования, вч-СРБ и ИЛ-6, измеренные в 1-е и 5-е сут, имели отрицательную корреляционную взаимосвязь с фракцией выброса (ФВ) левого желудочка (ЛЖ) и положительную корреляционную взаимосвязь с суммарным баллом дефекта перфузии (SRS). При этом показатель SUVmax не был статистически значимо взаимосвязан с уровнями вч-СРБ и ИЛ-6. В то же время SUVmax умеренно коррелировал с SRS (r = 0,517; р = 0,011). В моделях однофакторной линейной регрессии SUVmax не влиял на уровни вч-СРБ и ИЛ-6. В то же время SRS в покое оказывал статистически значимое воздействие на изменение уровней биомаркеров воспаления. Также стоит отметить, что корреляционная взаимосвязь между SUVmax и SRS отсутствовала.</p></sec><sec><title>Заключение</title><p>Заключение. Было показано, что интенсивность аккумуляции аналога соматостатина 99mTc-Тектротид в миокарде ЛЖ в раннем постинфарктном периоде не ассоциирована с уровнями биомаркеров воспаления. Полученные нами данные, с одной стороны, указывают на активацию клеточных и биохимических путей воспалительной каскада, с другой стороны, свидетельствуют о возможной противовоспалительной роли гиперэкспрессии соматостатиновых рецепторов 2-го типа.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Radionuclide imaging of somatostatin receptor type 2 (SSTR-2) shows great promise as a novel marker of cardiovascular inflammation. However, it is still unclear whether local inflammation and high levels of macrophages in the infarct area that overexpress SSTR-2 are positive or negative factors for myocardial healing.</p></sec><sec><title>Aim</title><p>Aim: To assess the relationship between inflammatory biomarkers as indicators of the systemic response to ischemic injury and the local inflammatory response in the myocardium, as measured by SSTR-2-targeted imaging, during the early post-infarction period.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. Twenty-three patients with acute primary anterior wall myocardial infarction and ST-segment elevation myocardial infarction (STEMI) were included in the study. On the first day (before percutaneous intervention) and on the fifth day after acute myocardial infarction (AMI), venous blood was collected from all patients to determine hsCRP and IL-6 levels. Five to six days after the acute coronary event, all patients underwent single-photon emission computed tomography (SPECT)/computed tomography (CT) with 99mTc-Tektrotyd. Seven days after the acute coronary event, they underwent myocardial perfusion scintigraphy with 99mTcMIBI at rest.</p></sec><sec><title>Results</title><p>Results. The results of the study indicated a negative correlation between hs-CRP and IL-6 levels on days 1 and 5 with left ventricular ejection fraction (LVEF) and a positive correlation with the summed rest score (SRS). Concurrently, SUVmax exhibited no correlation with hsCRP and IL-6 levels. At the same time SUVmax exhibited a moderate correlation with SRS (r = 0.517, p = 0.011). In the context of univariate linear regression, SUVmax demonstrated no statistically significant impact on hs-CRP and IL-6 levels. Concurrently, the total resting perfusion defect score (SRS) exerted an influence on the alteration in the levels of inflammatory biomarkers. It is also noteworthy that no regression relationship was identified between SUVmax and SRS</p></sec><sec><title>Conclusion</title><p>Conclusion. The findings indicated that the intensity of accumulation of somatostatin analog 99mTc-Tectrotyd in the left ventricular myocardium during the early postinfarction period is not associated with the levels of inflammatory biomarkers. Our findings indicate the activation of cellular and biochemical pathways of the inflammatory cascade.
