Липопротеин-ассоциированная фосфолипаза A2 (Lp-PLA2) рассматривается как независимый маркер сосудистого воспаления и прогрессирования атеросклероза. Эпикардиальная жировая ткань (ЭЖТ) активно участвует в патогенезе ишемической болезни сердца (ИБС) через модуляцию метаболизма. Потенциальная связь между уровнем Lp-PLA2, метаболическими параметрами и морфологией ЭЖТ у пациентов с ИБС остается неисследованной.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: изучить взаимосвязи между содержанием в крови липопротеин-ассоциированной фосфолипазы A2, состоянием липидного, углеводного обменов и морфометрическими показателями эпикардиального жирового депо, оцененными на клеточном и тканевом уровнях, у пациентов с документированной хронической ИБС.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включены 217 пациентов с ИБС. В крови определяли концентрацию Lp-PLA2, сортилина, глюкагона, C-пептида, глюкозы, HbA1c, липидный профиль, измеряли толщину ЭЖТ. У 42 пациентов оценивали размер адипоцитов ЭЖТ и степень их гипертрофии. Пациентов разделили на две группы по уровню Lp-PLA2: группа 1 (n = 205) – с концентрацией не более 95-го процентиля; группа 2 (n = 12) – выше 95-го процентиля.</p></sec><sec><title>Результаты</title><p>Результаты. У пациентов группы 2 выявлены более высокие уровни сортилина и глюкагона, увеличение толщины ЭЖТ, преобладание гипертрофированных адипоцитов. В группе 1 концентрация Lp-PLA2 статистически значимо коррелировала с концентрацией общего холестерина (ОХС), холестерина липопротеинов низкой плотности (ХС-ЛНП), глюкозой, HbA1c, средним размером адипоцитов ЭЖТ и долей гипертрофированных адипоцитов ЭЖТ. У мужчин группы 1 при наличии сахарного диабета концентрация Lp-PLA2 была статистически значимо взаимосвязана с индексом массы тела (ИМТ) и морфометрией ЭЖТ, у женщин группы 1 – с показателями липидного и углеводного обмена. Пациенты группы 1 со средним размером адипоцитов ЭЖТ более 89 мкм отличались более высокими уровнями Lp-PLA2 и С-пептида в крови по сравнению с теми, у кого средний размер адипоцитов ЭЖТ был меньше 89 мкм.</p></sec><sec><title>Заключение</title><p>Заключение. У пациентов с хронической ИБС уровень Lp-PLA2 связан с морфометрическими характеристиками эпикардиального жирового депо на клеточном и тканевом уровнях, а также с показателями липидного и углеводного обмена. Экстремально высокие концентрации Lp-PLA2 ассоциированы с увеличением толщины ЭЖТ, гипертрофией адипоцитов ЭЖТ, повышением уровней сортилина и глюкагона. Среди пациентов с умеренными значениями Lp-PLA2, в случае наличия крупных адипоцитов ЭЖТ, отмечен более высокий уровень C-пептида, а также выявлены гендерные различия: у женщин концентрация Lp-PLA2 была связана с параметрами липидного и углеводного обмена, у мужчин – с морфометрией эпикардиального жирового депо. Полученные данные подчеркивают значимость комплексной оценки системных и локальных метаболических маркеров, включая Lp-PLA2, для стратификации сердечно-сосудистого риска при ИБС.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Lipoprotein-associated phospholipase A2 (Lp-PLA2) is considered to be an independent marker of vascular inflammation and atherosclerosis progression. Epicardial adipose tissue (EAT) actively participates in the pathogenesis of coronary artery disease (CAD) through modulation of metabolism. The potential links between Lp-PLA2 levels, metabolic parameters, and EAT morphology in patients with CAD remain unexplored.</p></sec><sec><title>Aim</title><p>Aim: To investigate the relationships between blood Lp-PLA2 content, lipid and glucose metabolism, and morphometric parameters of the epicardial fat depot assessed at the cellular and tissue levels in patients with established chronic CAD.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. The study included 217 patients with CAD. Blood concentrations of Lp-PLA2, sortilin, glucagon, C-peptide, glucose, HbA1c, and lipid profile were determined. EAT thickness was measured. In 42 patients, EAT adipocyte size and the degree of their hypertrophy were assessed. Patients were divided into two groups according to Lp-PLA2 level: group 1 (n = 205), with concentrations not exceeding the 95th percentile; group 2 (n = 12), with concentrations above the 95th percentile.</p></sec><sec><title>Results</title><p>Results. Patients in group 2 had higher levels of sortilin and glucagon, increased EAT thickness, and a predominance of hypertrophied adipocytes. In group 1, Lp-PLA2 concentration correlated significantly with total cholesterol, C-LDL, glucose, HbA1c, mean EAT adipocyte size, and the proportion of hypertrophied cells. In men in group 1 with type 2 diabetes, Lp-PLA2 concentration was associated with BMI and EAT morphometry, whereas in women in group 1 with type 2 diabetes it was associated with lipid and glucose metabolism parameters. Group 1 patients with a mean EAT adipocyte size greater than 89 μm had higher blood levels of Lp-PLA2 and C-peptide compared with those with a mean EAT adipocyte size below 89 μm.</p></sec><sec><title>Conclusion</title><p>Conclusion. Lp-PLA2 level is closely related to the morphometric characteristics of the epicardial fat depot at both the cellular and tissue levels, as well as to lipid and glucose metabolism indices in patients with chronic CAD. Extremely high Lp-PLA2 concentrations are associated with increased EAT thickness, hypertrophy of EAT adipocytes, and elevated sortilin and glucagon levels. Among patients with moderate Lp-PLA2 values, the presence of large EAT adipocytes was accompanied by higher C-peptide levels, and sex differences were observed: in women, Lp-PLA2 concentration was linked to lipid and glucose metabolism parameters, whereas in men it was linked to EAT morphometry.
