Артериальная гипертензия (АГ) и ревматоидный артрит (РА) ассоциируются с увеличением риска сердечно-сосудистых заболеваний и их осложнений, включая симптомное и бессимптомное поражение головного мозга.</p><p>Оба заболевания оказывают синергическое воздействие на стенку сосудов, приводя к нарушению их компенсаторной способности. Ауторегуляция мозгового кровообращения регулирует тонус мозговых артерий в зависимости от колебаний артериального давления, но ее роль как фактора риска структурных изменений головного мозга остается недостаточно изученной.</p></sec><sec><title>Цель</title><p>Цель: изучить клинические и иммунно-воспалительные особенности течения РА у пациентов с АГ, проанализировать параметры эндотелиальной дисфункции, сосудистой жесткости, цереброваскулярной реактивности (ЦВР) и выявить ассоциации этих показателей со структурными изменениями головного мозга по данным МРТ.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В открытое обсервационное одномоментное поперечное исследование были включены 33 пациента с АГ 1–2 степени, коморбидной с РА, без неврологического дефицита в возрасте 61,0 [56,0; 65,0] лет. Всем пациентам выполнены МРТ головного мозга, транскраниальная допплерография средних мозговых артерий для оценки ЦВР в условиях гипероксической и гиперкапнической проб, объемная сфигмография для оценки сосудистой жесткости, ультразвуковая оценка эндотелийзависимой вазодилатации плечевой артерии (ЭЗВД) в условиях пробы с реактивной гиперемией.</p></sec><sec><title>Результаты</title><p>Результаты. Пациенты с АГ, коморбидной с РА, имели высокую частоту нарушения ЦВР в гиперокической и гиперкапнической пробах (72,7% и 87,5% пациентов, соответственно), ЭЗВД (81,8%), повышенной сосудистой жесткости (39,4%). По данным МРТ головного мозга у 100% пациентов выявили наличие расширенных периваскулярных пространств без признаков атрофии коры головного мозга. У 63,6% пациентов обнаружены очаговые изменения белого вещества головного мозга. Количество очагов в белом веществе головного мозга имело обратную корреляционную зависимость с дозой метотрексата (r=−0,84; p=0,0026), параметром ЦВР – нормализованным к артериальному давлению (АД) ответом резерва НОРО2 (r=−0,57; р=0,0297) в гипероксической пробе и прямую связь с коэффициентом изменения резистивного индекса в средней мозговой артерии при проведении гиперкапнической пробы КИRIСО2 (r=0,84; р=0,0003).</p></sec><sec><title>Заключение</title><p>Заключение. Структурные изменения головного мозга у пациентов с АГ и РА сопряжены с нарушением ЦВР, избыточной жесткостью сосудов, эндотелиальной дисфункцией и обратно связаны с интенсивностью противовоспалительной терапии. Полученные результаты позволяют определять ключевые терапевтические мишени при разработке стратегий нейропротекции у пациентов с АГ, коморбидной с РА.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Arterial hypertension (AH) and rheumatoid arthritis (RA) are both associated with an increased risk of cardiovascular diseases and their complications, including symptomatic and asymptomatic brain damage. Both diseases exert a synergistic effect on the vascular wall, leading to impaired vascular compensatory capacity. Cerebral autoregulation adjusts cerebral arterial tone to fluctuations in blood pressure, but its role as a risk factor for structural brain changes remains uncertain.</p></sec><sec><title>Aim</title><p>Aim: To study the clinical and immune-inflammatory features of RA in patients with AH, to analyze parameters of endothelial dysfunction, vascular stiffness, cerebrovascular reactivity (CVR), and to identify associations of these parameters with structural brain changes on magnetic resonance imaging (MRI).</p></sec><sec><title>Material and Methods</title><p>Material and Methods. The open observational cross-sectional study included 33 patients with grade 1–2 AH comorbid with RA, without neurological deficit, aged 61.0 [56.0; 65.0] years. All patients underwent brain MRI, transcranial Doppler ultrasonography of the middle cerebral arteries to assess CVR during hyperoxic and hypercapnic tests, sphygmography to evaluate arterial stiffness. Endothelial function was assessed using ultrasound evaluation of flow-mediated dilation (FMD) of the brachial artery.</p></sec><sec><title>Results</title><p>Results. Patients with AH comorbid with RA had a high frequency of impaired CVR in hyperoxic and hypercapnic tests (72.7% and 87.5% of patients, respectively), impaired FMD (81.8%), and increased arterial stiffness (39.4%). Brain MRI revealed enlarged perivascular spaces in 100% of patients with no signs of cortical atrophy. The majority of patients (63.6%) had focal white matter lesions. The number of white matter lesions was inversely correlated with the methotrexate dose (r =−0.84; p=0.0026), with the CVR parameter – the autoregulation response normalized by arterial blood pressure (NARO2) in the hyperoxic test (r=−0.57; p=0.0297). The number of white matter lesions was directly correlated with the relative change coefficient of the resistive index in the middle cerebral artery during the hypercapnic test (CCrelRICO2; r=0.84; p=0.0003).</p></sec><sec><title>Conclusion</title><p>Conclusion. The brain structural changes in patients AH and RA are associated with impaired CVR, increased arterial stiffness, endothelial dysfunction, and are inversely related to the intensity of anti-inflammatory therapy.
