Артериальная гипертензия является основной причиной сердечно-сосудистой заболеваемости и смертности. Мочевая кислота вовлечена в окислительный стресс, эндотелиальную дисфункцию и патофизиологию гипертонии. Исследование мочевой кислоты в слюне представляет собой неинвазивную, экономически эффективную альтернативу сывороточному анализу.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: сравнить уровень мочевой кислоты в слюне у взрослых пациентов с артериальной гипертензией и у здоровых людей с нормальным артериальным давлением, а также оценить диагностическую ценность этого показателя.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В проспективное исследование «случай-контроль» были включены 60 взрослых пациентов с артериальной гипертензией и 60 здоровых людей с нормальным артериальным давлением (контрольная группа), сопоставимых по возрасту и полу. Нестимулированная цельная слюна и стимулированная цельная слюна были собраны по стандартизированному протоколу. Концентрацию мочевой кислоты в слюне измеряли спектрофотометрически с использованием коммерческого набора QuantiChrom™ Uric Acid Assay Kit. Также оценивали мочевую кислоту в сыворотке, общую антиоксидантную способность (TAC) и скорость слюноотделения. Для статистической обработки данных использовали коэффициенты корреляции Пирсона и анализ ROC.</p></sec><sec><title>Результаты</title><p>Результаты. Уровень мочевой кислоты в стимулированной цельной слюне был достоверно выше в группе гипертоников (18,62 ± 3,27 мкг/мг белка) по сравнению с контрольной группой (6,14 ± 1,92 мкг/мг белка; p &lt; 0,001). Показатель мочевой кислоты в нестимулированной цельной слюне был аналогично повышен в группе пациентов с артериальной гипертонией. Уровень мочевой кислоты в стимулированной цельной слюне положительно коррелировал с систолическим артериальным давлением (r = 0,79; p &lt; 0,001), диастолическим артериальным давлением (r = 0,81; p &lt; 0,001) и уровнем мочевой кислоты в сыворотке (r = 0,84; p &lt; 0,001). Согласно ROC анализу, уровень мочевой кислоты в стимулированной цельной слюне имел самую высокую диагностическую эффективность среди проверенных параметров (AUC– 0,97, чувствительность – 92%, специфичность – 93%) для диагностики гипертонической болезни. Стимулированное слюноотделение было значительно ниже в группе гипертоников.</p></sec><sec><title>Выводы</title><p>Выводы. Мочевая кислота представляет собой перспективный неинвазивный биомаркер для скрининга и мониторинга гипертонии, требующий валидации в более крупных многоцентровых исследованиях.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Hypertension is a leading global cause of cardiovascular morbidity and mortality. Uric acid (UA) is implicated in oxidative stress, endothelial dysfunction, and hypertensive pathophysiology. Salivary UA offers a non-invasive, cost-effective alternative to serum analysis.</p></sec><sec><title>Aim</title><p>Aim: To compare salivary UA levels in hypertensive adults against normotensive controls and evaluate its diagnostic utility.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. This prospective case-control study enrolled 60 adult hypertensive patients and 60 age- and sex-matched normotensive controls. Non-stimulated whole saliva (NWS) and stimulated whole saliva (SWS) were collected following a standardised protocol. Salivary UA concentrations were measured spectrophotometrically using a commercial QuantiChrom™ Uric Acid Assay Kit. Serum UA, total antioxidant capacity (TAC), and salivary flow rates were also assessed. Pearson correlation coefficients and receiver operating characteristic (ROC) analysis were applied for statistical evaluation.</p></sec><sec><title>Results</title><p>Results. SWS UA was significantly higher in the hypertensive group (18.62 ± 3.27 µg/mg protein) versus controls (6.14 ± 1.92 µg/mg protein; p &lt; 0.001). NWS UA was similarly elevated. Salivary UA in SWS correlated positively with systolic blood pressure (r = 0.79, p &lt; 0.001), diastolic blood pressure (r = 0.81, p &lt; 0.001), and serum UA (r = 0.84, p &lt; 0.001). ROC analysis showed SWS UA had an AUC of 0.97 with sensitivity of 92% and specificity of 93% in differentiating hypertensive from normotensive individuals. Stimulated salivary flow was significantly lower in the hypertensive group.</p></sec><sec><title>Conclusion</title><p>Conclusion. Salivary UA, particularly measured in stimulated saliva, is markedly elevated in hypertensive patients and demonstrates strong diagnostic performance.
