У 124 пациентов (85 муж./39 жен., средний возраст — 50±12 лет), имеющих документированные тахикардии с широкими комплексами QRS по типу БЛНПГ, по результатам электрофизиологического исследования (ЭФИ) сердца был диагностирован различный генез источника аритмии. На этапе ретроспективного анализа (n=63) пациенты разделены на 3 группы: с желудочковой тахикардией (группа ЖТ, n=20), наджелудочковыми тахикардиями (НЖТ) с тахизависимой БЛНПГ (группа НЖТ+БЛНПГ, n=23) и НЖТ с проведением по дополнительным проводящим путям (группа НЖТ+ДПП, n=20). У каждого пациента анализировались 244 ЭКГ-параметра морфологии комплексов QRS. По результатам сравнительного, ROC- и многофакторного анализа различий между группами выявлялись наиболее ценные дифференциально-диагностические признаки, которые были объединены в диагностические ЭКГ-алгоритмы. На втором этапе исследования проводилась проверка точности ЭКГ-алгоритмов на проспективной группе больных (61 эпизод тахикардии у 57 больных).</p></sec><sec><title>Результаты</title><p>Результаты. ЭКГ-признаками, указывающими на желудочковый генез тахикардии с расширенными комплексами QRS по типу БЛНПГ, являются: 1) наличие начального зубца R в отведении aVR; 2) отсутствие расщепленной (М-образной) формы зубцов R в отведении I; 3) длительность зубца S в отведении V1 менее либо равное 100 мс. Для наджелудочковых тахикардий, имеющих аберрантные комплексы QRS по типу БЛНПГ, ЭКГ-признаками, указывающими на предвозбуждение желудочков по дополнительному правостороннему проводящему пути, являются: 1) длительность зубцов R в отведении V2 более 45 мс; 2) отсутствие расщепленной (М-образной) формы зубцов R в отведении I; 3) длительность зубца R в отведении aVL более 30 мс. Применение алгоритма дифференциальной диагностики ЖТ и аберрантных НЖТ с морфологией комплексов QRS по типу БЛНПГ позволило правильно диагностировать ЖТ с точностью 95% (чувствительность — 97%, специфичность — 92%). Применение алгоритма дифференциальной диагностики аберрантных НЖТ с формой комплексов QRS по типу БЛНПГ позволило правильно диагностировать НЖТ с предвозбуждением желудочков по правостороннему ДПП с точностью 84% (чувствительность — 65%, специфичность — 100%).</p></sec><sec><title>Выводы</title><p>Выводы. Полученные результаты позволили предложить новые критерии для дифференциальной диагностики тахикардий с расширенными по типу БЛНПГ комплексами QRS в дополнение к известным алгоритмам Велленса, Бругада, Гриффит и других авторов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate standard 12-lead electrocardiogram (ECG) indices for the differential diagnosis of left bundle branch block (LBBB) tachycardias.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. The study analyses 244 ECG indices in 63 retrospective patients (85 males and 39 females aged 50±12 years) with LBBB type tachycardias. The electrophysiological study identified ventricular tachycardias (VT) (VT group, n=20), supraventricular tachycardias (SVT) with LBBB (SVT+LBBB group, n=23) or antidromic SVTs (WPW group, n=20). Unifactorial, multifactorial, and ROC analyses were performed to develop diagnostic ECG algorithms. The prognostic accuracy of the algorithms was subsequently evaluated in a prospective group of patients with LBBB tachycardias (n=57).</p></sec><sec><title>Results</title><p>Results. ECG signs of LBBB VTs were as follows: 1) the presence of the initial R wave in the lead aVL; 2) the absence of a split (M-shaped) R wave in the lead I; and 3) the S wave duration in the lead V1≤100 ms. For antidromic LBBB tachycardias, the ECG signs were as follows: 1) the duration of the R waves in the lead V2≥45 ms; 2) the absence of a split R waves (M-shaped) in the lead I; and 3) the duration of the R wave in the lead aVL&gt;30 ms. The accuracy of the algorithm for diagnosis of VT with LBBB was 95% (sensitivity of 97%, specificity of 92%). The accuracy of the algorithm for diagnosis of antidromic tachycardias was 84% (sensitivity rate of 65%, specificity rate of 100%).</p></sec><sec><title>Conclusion</title><p>Conclusion.
