В исследование включен 231 пациент с диагнозом ТЭЛА, выставленным при поступлении в стационар. В дальнейшем пациенты были разделены на подгруппы в зависимости от верификации диагноза методом компьютерной томографии и наличия синдрома ХСН.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Резкое увеличение давления в системе легочной артерии (ЛА) способствует формированию и усугублению правожелудочковой систолической дисфункции, которая в условиях наличия ХСН приобретает большую распространенность и степень выраженности, что подтверждается эхокардиографическими признаками дисфункции правых отделов сердца: большими значениями линейных размеров правых отделов сердца, индекс объема правого предсердия составил 32,3 (29,4–37,9) мл/м2 против 29,1 (26,6–32,3) мл/м2 (U = 1583,00, p &lt; 0,001), большей распространённостью признаков гемодинамической нестабильности (удельный вес лиц с развитием обморока или пресинкопального состояния на этапе поступления в стационар 32% при наличии синдрома ХСН, против 16,7% при его отсутствии; χ2 = 4,49, p &lt; 0,05) и более значимым повышением лабораторных маркеров дисфункции и повреждения правого желудочка, входящих в модель прогнозирования неблагоприятного исхода ТЭЛА [BNP – 395 (204–789) пг/мл против 104,2 (38,9–212) пг/мл, U = 1162,00, p &lt; 0,001; высокочувствительный тропонин – 212 (194,5–297,6) нг/мл против 184,8 (168,0–194,8) нг/мл, U = 279,50, p &lt; 0,01; миокардиальный белок, связывающий жирные кислоты, – 0,37 (0,18–1,2) нг/мл против 0,2 (0,08–0,3) нг/мл, U = 530,00, p &lt; 0,001; копептин – 0,2 (0,05–0,92) нг/мл против 0,09 (0,04–0,15) нг/мл, U = 563,00, p &lt; 0,01; снижение функции почек]. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To identify the features of the structural-functional state of the cardiovascular system in patients with pulmonary embolism (PE) in the presence of chronic heart failure (CHF) syndrome.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. The study included 231 patients diagnosed with PE at admission to the hospital. Patients were further divided into subgroups depending on the verification of the diagnosis by computed tomography and the presence of CHF syndrome.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. An increase in pressure in the pulmonary artery system contributed to the formation and aggravation of right ventricular systolic dysfunction, which acquired a greater prevalence and severity in the presence of CHF. This was confirmed by the echocardiographic signs of dysfunction of the right heart including increased linear dimensions of right cardiac chambers, right atrium volume index of 32.3 (29.4–37.9) mL/m2 versus 29.1 (26.6–32.3) mL/m2 (U = 1583.00, p &lt; 0.001); greater prevalence of hemodynamic instability (proportion of patients with fainting or presyncope at hospital admission was 32% in the presence of CHF versus 16.7% without CHF; χ2 = 4,49, p &lt; 0.05); and more significant increases in the laboratory markers of right ventricular dysfunction and injury included in the prediction model of adverse PE outcome [brain natriuretic peptide of 395 (204–789) pg/mL versus 104.2 (38.9–212) pg/mL (U = 1162.00, p &lt; 0.001), high-sensitive troponin of 212 (194.5–297.6) ng/mL versus 184.8 (168.0–194.8) ng/mL (U = 279.50, p &lt; 0.01), myocardial fatty acid-binding protein of 0.37 (0.18–1.2) ng/mL versus 0.2 (0.08–0.3) ng/mL (U = 530.00, p &lt; 0.001), copeptin of 0.2 (0.05–0.92) ng/mL versus 0.09 (0.04–0.15) ng/mL (U = 563.00, p &lt; 0.01), and reduced renal function]. </p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>тромбоэмболия легочной артерии</kwd><kwd>венозные тромбоэмболические осложнения</kwd><kwd>хроническая сердечная недостаточность</kwd><kwd>сердечно-сосудистая система</kwd></kwd-group><kwd-group xml:lang="en"><kwd>pulmonary embolism</kwd><kwd>chronic heart failure</kwd><kwd>venous thromboembolic complications</kwd><kwd>cardiovascular system</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Статкевич Т.В., Патеюк И.В., Балыш Е.М., Петрова Е.Б., Моклая Е.В., Левкович И.Э. и др.
