Abstract

The iron metabolism disorders, such as iron deficiency anemia or iron overload, can modify the risk of malignancy and promote development more aggressive phenotypes of cancer. To investigate the association between iron metabolism and carcinogenesis we have examined 98 patients suffered from non-Hodgkin’s lymphoma. The different types of iron metabolism disorders in newly diagnosed patients are described: 32.6% of patients have cancer-associated high serum ferritin level, 41.7% – the anemia of chronic disease, 14.2% – latent iron deficiency and 2.1% – iron deficiency anemia. During chemotherapy the iron metabolism types’ distribution has been changed due to the fraction of patients with chemotherapy-induced anemia (38.8%) has appeared, whereas other types of iron metabolism disorders have been observed less frequently (18.38% of patients have the anemia of chronic disease, 18.4% – cancer-associated high serum ferritin level and 10.2% of patients have latent iron deficiency).

Abstract

Polymorphic loci of genes of folate metabolism C677T and A1298C MTHFR, A2756G MTR, A66G MTRR, G1958A MTHFD1 and 844ins68 CBS have been studied in patients with aggressive malignant Non Hodgkin lymphomas. The association of a rare allele 1958A MTHFD1 with a reduced risk of enlargement of diffuse large B-cell lymphoma (OR=0.429; C.I. [0.279–0.659], p<0.0008) is found. Impact of other polymorphic loci on susceptibility to enlargement of lymphomas has not been detected yet.

Abstract

It is known that rs78378222 disrupt TP53 gene expression and reduce the level of apoptosis in cells under the influence of genotoxic factors. Cases of rs78378222 detection in a tumor tissue have been described in DLBCL patients previously. Objective: to study the frequency of rs78378222 TP53 gene in tumor tissue of patients with diffuse large cell lymphoma (DLBCL) in Novosibirsk. Material for the study were the DNA isolated by phenol-chloroform extraction using guanidine from paraffin blocks of tumor tissue biopsies from 92 patients with DLBCL. Rs78378222 genotyping was performed by PCR RFLP analysis. The presence of the rare allele C of rs78378222 was performed direct sequencing of Sanger. rs78378222 in DLBCL tumor tissue was detected in 9.8% of cases. In same of the samples the marker was detected in the homozygous state, indicating the loss of heterozygosity in the TP53 gene in tumor tissue diffuse DLBCL. Rare allele C rs78378222 are not met in the homozygous state in healthy tissues. Thus, the phenomenon of loss of heterozygosity in the TP53 gene in tumor tissue of patients carriers rs78378222 with DLBCL is a classic example of the “two-hit” mechanism of carcinogenesis.

Abstract

Neutropenia is a common complication of infection caused by the human immunodeficiency virus (HIV). The etiology of neutropenia in this pathology can be quite different. This is the damage of precursor cells by human immunodeficiency virus (HIV), collapse of bone-marrow hemopoiesis caused by drugs for the HIV treatment and opportunistic infections, displacement of normal hematosis by tumorous HIV-associated cells and accelerated apoptosis of segmented neutrophils. These mechanisms are an important factor of developing opportunistic infection in HIV infected patients. Application of granulocytic colony-stimulating factors during complex therapy of HIV infection provides an opportunity to regulate the amount and the function of neutrophils, reducing the risk of consecutive infections. The article gives an overview of the literature and prospective studies of the author.

Abstract

The content of serum thymidine kinase in 229 patients with chronic lymphocytic leukemia (CLL), depending on the clinical course of the disease was studied. It was found that patients with CLL thymidine kinase has such a concentration which correlates with disease stage, indolent or progressive variants of the tumor process, a period of remission and relapse. Determination of thymidine kinase concentrations in patients’ serum at diagnosis provides an additional information for the prediction of survival, free from treatment, treatment response and overall survival of patients with CLL.

Abstract

We have analyzed the therapy outcomes of chronic myeloid leukemia of patients, received imatinib treatment for more than 12 month. Complete hematologic response (CHO) is attained in 98.7% cases (77 patients), complete cytogenetic response (CCR) (ph+<0%) – in 76.9% cases (60 patients), major molecular response (MMR) – in 58.9% cases (46 patients). The primary resistance to imatinib is observed in 15.4% cases (12 patients), secondary – in 7.7% cases (6 patients – 4 patients have lost the complete hematologic response (CHO) and complete cytogenetic response(CCR) and 2 patients have lost the complete cytogenetic response(CCR) and major molecular response (MMR)). The nine patients of those, who have primary or secondary resistance to imatinib have been treated with second-generation tyrosine kinase inhibitors (nilotinib, dasatinib) and in 88.9% cases (8 patients) the complete hematologic response (CCR) have been obtained, 5 patients (55.5%) have attained the complete cytogenetic response (CCR) and 4 patients (44.4%) have achieved the major molecular response (MMR). Among all those patients, who have been treated with I and II generations tyrosine kinase inhibitors, 83.3% patients have attained complete cytogenetic response (CCR), 64.1% – major molecular response. We have estimated overall survival (OS) analysis and obtained following results: the median of OS have not been achieved, 5-years OS rate is estimated as 90%, 10-years OS rate – more than 77%, calculated 15-years OS – more than 60%.

Abstract

We have measured the residual concentration of imatinib in blood serum of patients, who have been treated with generic imatinib (Philachromin) in 24±3 hours after last administration. Patients, who have residual serum blood level of imatinib higher 900 ng/ml, have attained complete hematologic response (CHO) and major molecular response (MMR) more often, than patients, who have lower level (p<0.05). To compare treatment outcomes of original imatinib (Glivek) and generic imatinib (Philachromin) as initial therapy for CML, we have obtained no statistical differences in frequency of achieving of complete hematological response. The likelihood of achieving the complete cytogenetic response (CCR) is higher in patients, treated with original imatinib, than generic (89.5 и 73.3% respectively), as well as the frequency of achieving complete molecular response (78.9 and 33.3% respectively, с<0.01). Adverse effects of generic imatinib have been observed more often, than adverse effects of original imatinib (66.6 and 44.7% respectively).

Abstract

The aim of the study was to investigate the state of oxidant-antioxidant system (OS-AOS) in patients with multiple myeloma (MM) during the autologous hematopoietic stem cells transplantation (autoHSCT). We studied 20 patients (11 men and 9 women, mean age 49 years) who followed autoHSCT after high-dose melphalan. The control group consisted of 50 age- and sex-matched healthy persons. The plasma levels of malonic dialdehyde and cerulloplasmin as well as activities of superoxide dismutase and catalase were measured by standard biochemical techniques. In erythrocytes, the level of non-protein thiol groups was studied. The state of OS-AOS was investigated in each patient four times: before and after conditioning with melphalan, at the moment of maximal leukocyte decrease and after complete reconstitution from cytopenia. We have found the features of impaired balance in OS-AOS in MM patients before as well as in course of autoHSCT. The level of malonic dialdehyde in MM patients was not significantly different from that in the control group. At the same time, cerulloplasmin plasma level as well as catalase activity were significantly increased in patient group (p<0.05), whereas the level of non-protein thiol groups was decreased in MM (p<0.05). The results of our study have shown, that an imbalance of OS-AOS is frequently seen in MM patients and, possibly, could influence the course of autoHSCT.

Abstract

The biochemical parameters of blood and quality of life were studied in patients with multiple myeloma and peripheral polyneuropathy during treatment with dynamic electroneurostimulation. 106 patients were divided into 3 groups: the 1st group received a standard chemotherapy with a dynamic electroneurostimulation course, the 2nd group was treated with a simulated placebo device, the third one used the standard treatment. In the first group the number of serotonin and dopamine increased in the blood plasma of patients in the 1.3-fold. The epinephrine and insulin number tended to decrease, while the norepinephrine and cortisol concentration increased by 1.2 and 1.3 times respectively. In the comparison group, these values have not changed. Physical functioning, general health and vitality, according to the SF-36, increased by 1.5, 1.7 and 1.5 times in the first group respectively, compared with baseline values that differ from the identical parameters in groups of comparison.

Abstract

The investigation of the structural-functional state of the left heart among patients with hemophilia has been made. 24 adult patients suffering from hemophilia A and B were surveyed. The control group was represented by 20 healthy men comparable in age. The use of the method of echocardiography contractile and diastolic functions of left ventricular was studied , the types of hemodynamics were detected, the phase analysis of functions of the left heart was made. Hyperfunction of the left ventricle in patients with hemophilia is primarily formed by an isometric type. The individual study of the nature of the left ventricular remodeling among patients with hemophilia showed that 63.8% have its normal geometry and 36.2% have its concentric remodeling. The defect of passive properties of diastole was noted. During the individual examination 39.7% of patients had a phase syndrome of the first-degree hypodynamia of the left ventricle and 60% had no definitive phase shifts on a chronocardiogram. During individual examination eukinetic type of haemodynamics (circulatory dynamics) was found among 65.5% and hypokinetic type of haemodynamics was found in 34.5% of patients.

Abstract

Long-term hematological syndromes were studied in patients with occupational pathology , conditioned by influence of hematotropic factors. At the moment of diagnosis hypoplastic anemia in combination with leukopenia and/or thrombocytopenia was detected in patients with chronic intoxication with organic solvents and chronic intoxication with uranium compounds. In chronic lead intoxication classic hypersideremic, with high reticulocyte count, hypochromic anemia was observed. In post-exposure period transient bi- or pancytopenia with predominance of normogenerative normochromic low-grade anaemia was found in all patients . In lead intoxication despite the fact that synthesis of porphyrines and haem have restored, the same inconstant anaemia was noted. Over the last 40 years no case of occupational health problem associated with blood has been registered.

Abstract

The review presents current information on the treatment of various diseases and secondary lesions of the digestive tract by a cytoprotective agents. It highlights the role of taurine, ursodeoxycholic acid, sucralfate, bismuth drugs and pentoxifylline in the treatment and prevention of diseases of the gastrointestinal tract. It is shown that the use of cytoprotectors greatly increases the efficiency of the basic regimens, which creates opportunities for the expansion of indications for their application and the search of new and more effective means of cytoprotection.

Abstract

On the basis of the morbidity rate of various forms of acute leukemia was determined for the 10 year post-hoc analysis in Novosibirsk and Novosibirsk region. immunophenotypic characteristic of acute leukemia of patients living on this territory was made. The structure of genetic and molecular genetic abnormalities occurring in blast cells in case of acute myeloid and lymphoid types of leukemia was surveyed. The Correlation of recurrent variants of acute leukemia in accordance with genetic and biomolecular criteria of The World Health Organization (WНO) classification was determined. The meaning of data of molecular genetic (FISH and mycroarrey genochipping) research studies for scoring perspective of treatment and prognosis of disease was shown.

Abstract

Vascular density was studied in the lymph node biopsies from 28 patients with Castleman’s disease (hyaline vascular and plasma cell types) with the help of digital histomorphometry. The method allowed for quantitative assessments and statistical analysis. Plasma cell type demonstrated the highest vascular density. The data on hyaline vascular Castleman’s disease showed a high statistical spread, suggesting heterogeneity of this particular type of the disease.

Quality examination of medical care in hematology and oncology – are an effective control method for the provision of specialized medical services to the population. Quality defect structure can effectively identify common problems of the health care system organization in this area. Abstract

People‘s motives of participation in donorship is a point in understanding how it is necessary to make a regional communication programs of donorship propaganda. A series of research dedicated to study of the structure of donors and theirs motives to blood donorship among people living in Novosibirsk were made. The authors made a conclusion about altruistic state of mind of donor population of Novosibirsk region and suppositions of developing of volunteer donorship in the region, based on the research data.

In the year 2007 the reorganization of blood supply service in Novosibirsk region was made. As a result of the reformation, the number of the institutions procuring donor blood reduced in 5 times, authorized staff size – in 2 times.
